Your Waiver / Intake Form

I understand it is the parent/guardian/adult responsibility to notify the Chosen Footsteps/Shining His Light Representative of the decision regarding picture/video/social media use. Individuals who do not approve will receive a special identifier throughout the experience.*

Chosen Footsteps LLC and Shining His Light Nonprofit welcome participants, families, and groups to take their own photos/videos during any experience.

I understand that Chosen Footsteps cannnot control or monitor photos or videos taken by other guests in public areas. By participating, I acknowledge that other attendees may take pictures or recordings, and Chosen Footsteps is not liable for photos or videos taken by other guests without individual knowledge or consent.*

I understand the individual may participate in activities that include movement, crafts, interactive experiences, and faith-based lessons. I acknowledge that participation may involve minor risks such as slips, bumps, or scrapes, and I voluntarily assume all risks associated with these activities.*

I release and hold harmless Chosen Footsteps, LLC, Shining His Light Nonprofit, it's staff, volunteers, and affiliates from any and all claims injuries, or damages that may arise from the individual participation in any program, event, or activity. *

I authorize Chosen Footsteps staff or volunteers to administer basic first aid and seek emergency medical care for the individual if necessary.*

I understand each individual is expected to follow instructions and behave safely during all activities. If unsafe behavior occurs, Chosen Footsteps may remove the individual from the activity for their safety and the safety of others.*
